NEW PATIENT FAQs

What is the cost of becoming a member?  What is included in each of the Wellness Programs?

I’m not sure yet.  Can I talk with Dr. Caccopola before enrolling?

First, we recommend completing the FREE ONLINE HEALTH ASSESSMENT (go to GET STARTED page).  The link will bring you to LivingMatrix Functional Medicine software to create a HIPAA-compliant profile. There is no obligation to enroll as a patient member with its completion.

 

Within 2 business days, Dr. Caccopola will provide a Functional Medicine approach to your reported symptoms via email, as well as, an initial recommendation on Wellness Programs.  

 

After completing the Free Online Health Assessment, we recommend all patients take advantage of a FREE 15-min Phone Session with Dr. Caccopola before enrolling.  This phone session can usually be scheduled via phone or email quite readily. It provides an opportunity for you to ask questions and determine if Heightened Health is a good fit for your medical needs. 

What's a "DISCOVERY CONSULTATION"?

Even after completing the Free Online Health Assessment and 15-min phone session, some prospective patients desire a more-indepth, face-to-face medical consultation to obtain a clearer sense of Dr. Caccopola's approach to their individual medical conditions before enrolling as an active member.   

  • Discovery Consultations are allowed one-time, pre-enrollment.

  • Fee: $450 for 45 min visit per individual; includes review of your Complete Medical History forms completed prior to visit

  • Complete medical history forms must be completed before scheduling this consultation.  

  • Such consultations do NOT include physical exam.

  • Dr. Caccopola will answer any questions and provide feedback to your specific health questions or lab results.

  • If prospective patients desire medical records/labs be reviewed before consultation, then a charge of $75 per 15 min of  review time will be added to the consultation fee. 

  • This time is reserved specifically for you. Please provide 48 hrs notice to avoid $225 cancellation fee.

  • Missed time or late arrivals will still be charged full amount.

 

I'm ready to enroll.  How do I become a member of Heightened Health?

Go to the ENROLL NOW subpage (under "GET STARTED".)  Click on the desired Wellness Program.   If you haven't completed your Free Online Health Assessment or spoken with Dr. Caccopola yet, we recommend doing so before enrolling.

A valid bank account for ACH withdrawal is required (credit cards are also accepted, but not preferred).  Your account will not be charged until your first appointment. 

 

Multiple family members can be enrolled into the same Wellness Program but please call us directly to enroll family members into different programs.

 

Do you offer membership discounts to families?

Yes.  Families are defined as 2 or more relatives living within the same household, who are registered under a single payer account.  Enrollment fees for families are capped at $450.00.  In addition, each family member's monthly fee is given a $5 discount if enrolled in the Wellness Promotion Program and a $15 discount if enrolled the Wellness Restoration Program.

I just enrolled. When can I schedule my first appointment?

After enrollment, you will be sent an onboarding email with instructions to complete your New Patient Medical History Forms. These forms must be completed before we can schedule your New Patient History and Physical.  

 

How long are appointments?

New Patient History and Physical appointments are typically 90 minutes for adults and 60 minutes for children. The last 15 minutes of each appointment are reserved for visit summary, treatment recommendations, supplement/medication fulfillment, and lab draw. 

 

As our New Patient visits are quite comprehensive, we do not routinely perform gynecological exams during these times.  If you are in need of routine gynecological/breast exam, please let us know so we can schedule a dedicated visit.   

 

The Follow-up Visit to the Initial New Patient Exam is typically 45 min and includes the review of initial labs and execution of your health action plan.

 

Other patient visits can range from 15-75 minutes, depending on acuity and complexity.  Again, the last 5 -10 minutes of each appointment are reserved for visit summary, treatment recommendations, supplement/medication fulfillment, lab draw, and check out.

Annual History and Physicals for established patients are typically 60-75 min. 
 

Is my doctor really on-call 24/7?

Dr. Caccopola is on-call after-hours for Heightened Health members daily for urgent/emergent issues. If Dr. Caccopola is unavailable due to a holiday, vacation, or other situation, she will make appropriate arrangements to have another reliable healthcare provider cover for her or she will refer you to the ER/Urgent Care.

 

How can I contact my doctor for non-urgent issues?

Once you are a Heightened Health member, you will be invited to connect to our secure, HIPAA-compliant, mobile-friendly, patient portal, ElationPassport.  You can then readily communicate with Dr. Caccopola for non-urgent issues via ElationPassport.  These may include brief questions about your health, medication refills, and non-urgent appointment requests. 

 

Dr. Caccopola makes every effort to respond within the same or next business day unless a weekend, vacation, holiday, or other situation prevents a more timely reply.  If you have an urgent need, please call the office directly at (847) 868-0484.

 

How can I get my medications refilled?

Once you are an active member, medication refill requests can be submitted via our HIPAA-compliant, mobile-friendly, patient portal called Elation Passport. Dr. Caccopola makes every effort to complete your request within the same or next business day unless a weekend, vacation, holiday, or other situation prevents a timely reply.  After-hours telephone requests for routine medication refills will not be granted.

 

Do you perform minor office procedures?

Yes. Please refer to OUR SERVICES and Wellness Programs Chart Page 2 and 3 for more specifics.

Do you treat chronic pain?

Dr. Caccopola does not manage chronic pain medications.  She treats chronic pain with the aforementioned non-pharmacological approaches.  If you are in need of chronic pain management through the use of controlled substances, like narcotics, she will refer you to the appropriate pain specialist.

 

Do you treat mental health conditions?

Dr. Caccopola does not manage controlled substances related to mental health conditions, such as ADD/ADHD, Anxiety, Bipolar, Major Depression, or Substance Abuse. She will maximize non-pharmacological strategies and work with your psychiatrist so that he/she could potentially minimize your medications.

 

Do you provide routine childhood vaccinations?

Due to our low patient volume, we do not carry routine childhood immunizations. Please call to discuss.

 

Do you provide emergency medical care and treatment?

No. Dr. Caccopola does not provide diagnosis or treatment of emergent or life-threatening conditions; she recommends calling 911 or going directly to the nearest emergency room.

 

Do you perform surgery?

No. If you are in need a surgical procedure, you will be referred to the appropriate surgical specialist for evaluation. 

Do you round on patients in the hospital, assisted living, rehabilitation, or nursing home facilities?

Dr. Caccopola is committed to an office-based practice only. However, she does partner with excellent hospitalists, specialty groups, and staff physicians, with whom she can coordinate your care.

Do you work with small businesses that want to provide health care to their employees?
