  • Up to five (5) routine follow-up Wellness Exams per enrollment year available 

    • Teens: up to three (3) routine follow-up exams â€‹

  • Wellness Exams may be used for:

    • New Pt Appt (60 min)

    • Annual Exam (45 min)

    • Follow up Visits (30-45 min)

    • Telehealth Visits: (25 min)

    • Acute Visits (time varies)

    • Health Coaching Visits

    • Female/Male Health Exams

    • Hormone-related services

    • School/Sports Physicals

    • Minor Office Procedures

    • Conventional lab/test review

CONSULTATION

For prospective patients

who desire in-depth, face-to-face consultation with Dr. Caccopola before enrolling.

Allowed one-time, pre-enrollment.

​

Adult or Child: $540

per individual

Includes: ​

  • One (1) face-to-face visit (45 min) with pre-visit preparation​ (45 min)

​

  • Comprehensive Medical History Forms must be completed before scheduling this appointment.

​

  • Forms will be reviewed prior to your evaluation. 

​

  • Our doctor will answer any questions and provide specific feedback to your individual health inquiries and lab results.

​

  • Does not include physical exam or lab draw.

​​

  • Any review of medical records will be done before consultation; such review will be charged at $90 per  15 min and added to the fee.

​

​

Other:

  • If patient subsequently enrolls into a Wellness program, then $270 credit will be applied to offset initial enrollment fee.

  • This time is reserved specifically for you. Please provide 48 hrs notice to avoid $270 cancellation fee.

  • Missed appointments or late arrivals will still be charged full amount.​

**Our healthcare professional is available 24/7 for ongoing Wellness Exam support for established patients of Heightened Health. Urgent/emergent medical issues will be referred to Emergency Room or Urgent Care.  There may be times that Patient cannot contact our Practice healthcare professional due to vacations or illness, or due to technical defects with either Patient’s or Practice’s electronic communication equipment. Should our Practice healthcare professional become unavailable, the Patient must call 9-1-1 and/or seek emergency medical attention.
